Effective January 1, 2018, to prevent unauthorized copying, altering and forgery of medical marijuana authorization forms, every authorization written in the state of Washington by a licensed practitioner must be written on 8.5” x 11” tamper-resistant paper approved by the Washington State Pharmacy Board with the Washington State Medical Marijuana RCW seal:
• Seal consists of the applicable law reference of RCW 69.51A.030
• Seal is printed in blue (RGB 0, 33, 87; CMYK 100, 92, 34, 35)
• Seal is located in bottom right of the authorization form (see attached Word document for placement)
Washington has approved the use of board-approved tamper-resistant paper, with the RCW seal, to be printed in the office. Prescribers are responsible for safeguarding medical marijuana authorization paper from theft.
Washington Medical Marijuana Authorization form blank must have at least one of the following security features:
• “Void” pantograph if photocopied
• Security features printed on front
• Security watermark on back states “Security Authorization”
• Chemical-reactant stain appears to make prescription form unusable if chemically altered.
Note: This is ONLY for Medical Marijuana Authorizations and does not apply to any other prescriptions.
